RADICULAR BACK PAIN CARE
Radicular low back pain – the back pain that radiates down the leg – is often the most aggravating. Researchers observed that chiropractic spinal manipulative therapy and benzodiazepine drugs were frequently prescribed. They questioned how spinal manipulation affected the use of the benzodiazepine drugs in patients aged 18-49 years old. The risk of receiving a prescription for a benzodiazepine were significantly lower for those undergoing chiropractic spinal manipulation. The risk did go up a bit as the back pain lingered: 3 months .56, 6 months .61, and 12 months .67. The researchers also questioned whether a patient’s preference to avoid prescription medications affected this. Their sensitivity analysis did not have anything to do with the risk. The study closed by saying that chiropractic spinal manipulation care significantly decreased the odds of a prescription for and affected the use of benzodiazepine for radicular low back pain. The patients in the study who began with the chiropractic manipulation path were more apt to get published guideline-directed care for benzodiazepine prescription. OPIOID PRESCRIPTION FOR BACK PAIN
Back pain sufferers with acute onset, preliminary episode of back pain commonly go to the emergency room for help. Opioid prescribing in the ED was reported as not always adhering to clinical guidelines which recommended limited use. In 2016, researchers reported that such early opioid prescribing for uncomplicated acute low back pain increased the long-term use of opioids as well as the cost of care notwithstanding the fact that there are a variety of adverse outcomes linked to opioid use. RECOMMENDED BACK PAIN TREATMENT OPTIONS

(3) Spinal manipulation was a recommended first-line treatment for low back pain in adults. 97%-98% of spinal manipulation providers were chiropractors. (4) A 2021 follow-up report on whether the recommendations were followed showed some interesting facts. A Gallup poll of 5377 random people resulted in 545 low back pain sufferers who went to a medical doctor in the past year. They were asked what type of care their medical doctor recommended and if they followed that care. 96% said they were given a recommendation of one or more pain treatments. 81% said their medical doctor recommended drug and non-drug therapies. 76% - NSAIDs and acetaminophen. 79% - self-care treatments. 37% - massage, acupuncture, or spinal manipulation. 60% - physical therapy. Nearly 66% said prescriptions (benzodiazepine, opioids, gabapentin, Neurontin, cortisone injections) were recommended.
